You must be 18 or older, live in North Carolina, and meet the BMI criteria. You do not need to be a current Eleanor member or enrolled in any other Eleanor program to qualify.
You may qualify with a BMI of 30 or higher. If your BMI is between 27 and 29.9, you may still qualify if a doctor has diagnosed you with at least one of the following: high blood pressure, high cholesterol or triglycerides, cardiovascular disease, heart disease, heart attack, or sleep apnea.
Yes. After you submit your intake, our medical team will review your health history and order the right labs from the LabCorp or Quest nearest you. There is nothing you need to arrange on your own.

Your physician will determine which medication is right for you based on your health history and care plan.
Yes. Eleanor has partnered with Culina Health to provide Registered Dietician and nutrition counseling services. Culina Health accepts most insurances and can be connected to your care plan by your Eleanor physician.
