Recovery at Eleanor Health

Our Care Model

Starting Strong: Month 1

Your journey begins with a comprehensive intake during your first visit. You’ll meet with a medical provider to assess your symptoms and begin care planning to help define your treatment goals.

Over the course of the month, you will meet with the following team members based on your needs:

  • Medical Team Provider
  • Nurse
  • Therapist
  • Community Recovery Partner
  • Care Manager

Our research shows that members benefit most from more frequent visits in the first month of care. We recommend care team visits three times a week for best support.

Best Support: 3 visits/week

Strengthening Support: Month 2 -3

We adjust how often we see you and the support we provide to fit your schedule and your progress on your recovery journey.

Best Support: 1-2 visits/week

Recovery for Life: Month 4+

We track progress towards your goals together during visits, and you can also monitor it through your Member Portal. We recognize that your needs may change along your journey, and we will adjust your treatment to meet you where you are.

Eleanor Health currently accepts members in ColoradoFlorida, Idaho, Louisiana, MassachusettsMissouri, Montana, New Jersey, New Mexico, North Carolina, Ohio, Pennsylvania, Texas, and Washington (Illinois coming soon). Our services are available remotely from anywhere in the state.

You can do your entire treatment online, including getting a prescription for suboxone or other FDA-approved addiction medications to stabilize your physical health. From the beginning, you will work on getting better with our counselors and peer recovery specialists.

The first visit includes a comprehensive intake with a medical provider to review symptoms, current substance use, health history, and recovery goals. The outcome is a personalized care plan that can include medication, counseling, and recovery support.

Care is delivered by a multidisciplinary team that can include a medical provider, a nurse, a therapist, a community recovery partner (peer support), and a care manager. Your team mix depends on your needs and goals.

The first month is structured for momentum. Visits are more frequent while treatment is being established, symptoms are stabilized, and goals are clarified. A common recommendation is about three care team visits per week during this phase.

Early recovery often includes changing symptoms, cravings, and stressors. More frequent touchpoints help with medication adjustments when needed, skill-building, accountability, and faster problem-solving when challenges come up.
