Health Advocate (HA)

Health Advocates assist members with social needs and connect individuals to essential resources. They are skilled in gathering and providing resources to help members overcome social barriers to health and wellness. Whether it’s housing, finances, transportation, or prescription assistance, HAs ensure that members have access to the necessary support to reach their health goals.

At Eleanor Health, community members have access to various forms of therapy and counseling, including:

  • Individual therapy

Doctor/Provider

If your care plan involves Medications for Addiction Treatment (MAT), you will meet with a doctor or provider. MAT is a proven approach to managing cravings and withdrawal symptoms in substance use disorder. It may include medications like Suboxone for opioid addiction, or Campral for alcohol-use disorder. The doctor or provider will discuss your care plan, recommend the appropriate medication, and closely monitor your progress. The goal of MAT at Eleanor Health is to help you:
