Meet the Cherry Hill, NJ clinical team!

My name is Chris Borkowski, and I come to Eleanor Health as the Cherry Hill Practice Administrator. I have been working with individuals who struggle with mental health and substance use disorders for the past 15 years. I am excited and honored to help bring Eleanor Health’s innovative care model to the great state of New Jersey. When I am not working, I enjoy spending time with family and friends, hiking, golfing, and playing games (video and board).
I am excited to introduce you to the rest of the Eleanor Health Cherry Hill care team!
Shane Pomales, Community Recovery Partner
My name is Shane, and I am a Certified Peer Recovery Specialist, Certified Kundalini Yoga instructor, and have spent years helping individuals with developmental disabilities. He believes that there is nothing more rewarding than being able to use his unique experience in recovery to help our members live healthy and fulfilling lives. Shane keeps a positive attitude by participating in hobbies that he enjoys such as: archery, gaming, model painting, and attending heavy metal concerts.
Kaya Daley, LCSW
My name is Kaya Hazard-Daley, LCSW. I am the Lead Clinician at the Eleanor’s Cherry Hill, NJ Site. I am a native of Massachusetts where I earned my Master’s in Social Work. I have several years of experience working as a provider in Outpatient Behavioral Health facilities with both military members and their families, as well as in civilian settings. I am a passionate social worker. When I am not working, I enjoy being with my two small children, reading, or watching a movie.
Suzanne Bracchi, RN
My name is Suzanne Bracchi, RN, and I come to Eleanor Health as the Nurse Care Manager. I bring with me over ten years of experience working with individuals impacted by mental health and substance use disorders. I am excited to support our community and help individuals live amazing lives through treatment and recovery. I am inspired by nature and enjoy spending time gardening and hiking. One of my favorite quotes is from Eleanor Roosevelt: “It is better to light a candle than curse the darkness.”
Melinda Riedel, Member Engagement Administrative Specialist
My name is Melinda Riedel, my role with Eleanor is in the MEAS position. I have the honor of being one of the first people that our members will interact with upon joining our team and each additional encounter thereafter. I have been in healthcare for 5 years and I have extensive administrative experience. In my past, I have spent time being trained at a crisis intervention hotline as well as volunteering there. I attended school for Behavioral Health with a focus on Addiction Studies and look forward to my future here at Eleanor Health. If I am not at work, or with family and friends, you can find me out in nature, primarily at the beach for some long-distance walking or taking an impromptu road trip to somewhere I have never been before. Other hobbies include cooking, travel planning, and visiting historical sites.
If you need help with your substance use disorder, we are here to help you build your confidence and momentum towards the future you want. We provide treatment services for adults with alcohol, opioid, and other substance use disorders. We are conveniently located in Cherry Hill, Edison, and Verona, New Jersey. We also provide services via Telehealth.
